1. Fill out the form below for quick access to the Dexter INDEX. Please use your new Dexter Distribution or previous account number. 
2. All fields below are required except Invoice Number. If you include this, it must be dated within the last 120 days. 
3. Click Here for help finding your Dexter Distribution account or invoice numbers.
4. Submit your user information to access all the great features Dexter INDEX offers.
